Forklift operation– (QLD May 2010). The purpose of the safety alert is to remind employers and forklift operators of the need to operate forklifts in a safe manner. A recent fatality in Queensland has highlighted the need for employers and operators to ensure forklifts operations and processes meet best practice
 
Go-kart– (NSW April 2010). A fatal accident at a New South Wales go-kart track has prompted a safety alert, recommending people do not go for a ride if their hair or clothing is not properly secured. The 26-year-old Sydney woman died earlier this month when her Islamic head dress got entangled in the rear axle of the go-kart at a Port Stephens track.
 
Foil ceiling insulation– (NT March 2010). Electrical workers & contractors, tradespersons and householders must be aware of the electrical safety risks associated with ceiling spaces where foil insulation has been laid.

These documents are similar to the Work Instruction as they provide the “How To” methodology but the document has an important distinction from any other. It identifies the Hazards, Assesses and rates the Risks, categorises the Risk into 4 classes and offers the Control method.

We'll start by providing an overview of Section 1, where the page of the S.W.M.S. is broken into various headings.
  • The Task to be performed
  • The representative that has prepared the S.W.M.S.
  • The client acceptance signature
  • The Skills, Training & Plant Requirements for this task

Under Section 2, this first page highlights;

  • The stage of activity – in this case Pre Commencement
  • The Risk Class – High, Medium or Low
  • 4 Risk Categories – Safety, Quality, Environmental and Financial
  • The Risk Control Methods
  • The introduction of “Hold Points” – project must not proceed until sign off.

Depending on the particular project there will be several pages involved in this section.

The second page deals with the related documents that need consideration for this task, such as;

  • Engineering Certificates, Government approvals etc,
  • Regulations, Advisory Standards, Codes of Practice, etc
  • Associated procedure and work instruction documents

Section 3 continues to identify;

  • The Plant and Equipment which will be used and
  • The safety and maintenance frequency.

The final section 4 covers the requirement for all persons working on this task to be identified and sign off to having the skills and training necessary to complete the work.
