Forklift operation– (QLD May 2010). The purpose of the safety alert is to remind employers and forklift operators of the need to operate forklifts in a safe manner. A recent fatality in Queensland has highlighted the need for employers and operators to ensure forklifts operations and processes meet best practice
 
Go-kart– (NSW April 2010). A fatal accident at a New South Wales go-kart track has prompted a safety alert, recommending people do not go for a ride if their hair or clothing is not properly secured. The 26-year-old Sydney woman died earlier this month when her Islamic head dress got entangled in the rear axle of the go-kart at a Port Stephens track.

We believe that workplace related incidents are caused, do not just happen and therefore can be prevented. Furthermore it is everyone’s responsibility to play an active part in the prevention.

This Incident Triangle is a familiar favourite for explaining to staff if the 600 incidents where no injury or damage occurred, was correctly reported and investigated it can prevent;

  • Damage
  • Minor Injury
  • Serious or Fatal injury.

At MINC Risk Services our aim is to help companies take a proactive approach similar to the message given from this diagram.

When conducting Incident Investigations and analysing the results we have a number of primary aims;

  • To determine how similar events may be prevented in the future.
  • To gather the facts and group them with other incidents to identify if patterns exist.
  • To communicate the findings and make staff aware of the hazards.
  • To assist in setting priorities for preventative measures
  • To provide a basis for evaluation of the effectiveness of those preventative measures.

There are also a variety of definitions that best describe an Incident. Here’s several for consideration.

  1. An occurrence or event that interrupts normal procedure
  2. An deviation from standard
  3. An undesired event that could cause downgrade to the efficiency of the business operation.

Incidents types can be classified under the following headings;

    • Dangerous events
    • Serious body injury
    • Work related illness
    • Work injury
    • Workplace incident
    • Near hit or miss
    • Accident

Whilst the Incidents types are straight forward MINC Risk Services can assist you to break down the Incident itself to find the root cause. Depending on the situation and circumstance we will adopt the most practical methods during the investigation.

In general however the process breaks down the Incident into 4 broad categories and explores each of these in detail. These are identified as;

  1. The Incident Results
  2. The Incident
  3. The Immediate Causes of the Incident
  4. The Root Causes of the Incident

Using these four categories MINC Risk Services will commence with The Incident Results and work backwards through the events until reaching the Root Cause/s.

On conclusion of the investigation MINC Risk Services shall provide the client with a written report which outlines;

  • The facts identified which surround the event
  • The Event process flow including contributing factors
  • Cost Consideration Guideline Table which aids managers to identify the real costs associated with the incident.
  • Findings summary including Hazards Identification and prioritised Risk Assessment
  • Recommendations for implementing prevention measures

The “Iceberg Principle” is well used in our approach to calculating the true costs of Workplace incidents

In our example we will examine the obvious costs that sit above the surface for all to see. Things like;

  1. The Injury costs
  2. The Property damage and repair costs
  3. The cost of rehabilitation

But it is the hidden charges which are often over looked but every bit as critical. Such as;

  1. Production losses
  2. Resources drain
  3. Re-training expense
  4. Idle machine time
  5. Investigation costs
  6. Impact on morale
  7. And many, many, more….
